Long-lived process that owns the :conduit_mcp_sse_streams ETS table,
the slot table behind the HTTP transports' :max_streams cap on
streamed POST responses: one row per open stream, keyed by the pid
serving it.
Started under ConduitMcp.Supervisor by ConduitMcp.Application, so the
table outlives every individual stream.
